What is EAP ?

E- Employee A- Assistance P- Program is a special program designed by MACIAF and Madison Center and Hospital to provide a confidential service to members with personal problems - problems which may be affecting your work performance, your relationships with others, your family.

Why Do We Have it at MACIAF?

MACIAF's management believes that EAP is an important benefit to offer members. They know that at any given time, some of their members will be troubled with personal problems - not because MACIAF has bad, weak, or inefficient members, but because over a lifetime, most people encounter personal difficulties at one time or another. Often, these problems are solved over time. That's why MACIAF is offering this program to you.

What kind of Problems
Can MBA-EAP Help With?

The major problem areas that EAP can help with are:

Alcoholism
Drug Abuse and Dependency
Stress
Depression
Family Problems
Marital Conflicts
Child and Adolescent Problems

Madison Center and Hospital's professional staff provides a wide range of therapeutic services.

How Does MBA-EAP Work?

There are two ways to put the MBA-EAP to work for you:

Self-Referral:w of problems that you may be having,

Supervisor Referral: There may be times when a supervisor notices that a member's work performance has declined. He

In either case, you begin to use your MBA -EAP  by calling Madison Canter and Hospital where you will first be seen by an admissions representative. An initial evaluation will be made, after which you will meet with a Madison Canter and Hospital counselor or be referred to other service providers, when necessary for treatment. Arrangements will be made for you to continue seeing the counselor, if needed, who will work with you to resolve your problems.

Will it Jeopardize My Job?

Participation in the MBA-EAP will not jeopardize your job security nor will it affect future promotional opportunities.

MACIAF believes that emotional problems, alcoholism, drug abuse or their life adjustment problems can be treated and resolved.

If you have been referred to MBA-EAP by your supervisor because of problems at work, the Employee Assistance Program will work in conjunction with the company's personnel policies to improve the work performance. The program is not a substitute for normal disciplinary procedure.

Confidentiality

Confidentiality is one of the most important aspects of the MBA-EAP program. If you contact Madison Center and Hospital directly, no one will know - MACIAF will merely be informed as to how many members are participating in the program, not who the members are.

If your supervisor refers you, Madison Center and Hospital will ask you to sign a release to let your supervisor know whether you have agreed to participate, and whether you are continuing to attend treatment sessions. No information concerning the nature of your problems will be released without your consent.

How Much Will it Cost Me?

MACIAF will pay the cost of the initial assessment session and five additional sessions. (NOTE:This benefit allows for six sessions of counseling per family member per year - which includes the member, spouse and eligible children.) If your counseling goes beyond these six sessions, you will be expected to pay for further sessions. Member health insurance may pay for a portion of your treatment. Madison Center and Hospital uses a sliding fee scale to determine the charge for sessions. Your fee will be based on total yearly income and number of dependents.

How to Start Using Your MBA-EAP

You can start using MBA-EAP right away by calling Madison Center and Hospital at 234-0061. You must have your Union card with you as well as documentation of your income.
